Please choose the following link below that fits your teen education status for the classroom and behind the wheel training. Each packet consist of all enrollment requirements that must be completed for the students to attend class. Notice you can either pay on-line or mail in your registration fee to reserve your seat in one of our classes, but all forms must be turned in the first day of class. Click on the locations and classroom schedules link on the left to see all classes offered, and there you can choose to pay on-line as well. Students can sign up as early as 14 years old but they must turn 15 before the last day of class. Please give us a call if you have any questions.

For you convenience we have placed links to all the forms that are required at the time of registration. Just click on the form you wish to print. Please give us a call if you have any questions about what form to use. We have also added a link if you don’t have Adobe Reader here is your free link.
This form is a wavier and must be signed by the superintendent of the school. This will allow students who are not longer attending school to enroll into either behind the wheel or the classroom portion.
Behind the Wheel Training Only
The Behind the Wheel Only is for students that have passed the classroom portion. All students must complete the forms below and provide a copy of their report card showing they completed the class with a passing grade.
